Menstrual colics, Painful menstruation, Menstrual pains (Dysmenorrhea)

The menstrual colic is the contraction of the uterus when you menstruate by the hormonal imbalances; it’s a local pain and a general discomfort. The hormonal imbalances are due to an excessive production of the hormones prostaglandins during the ovulatory period and an imbalance between estrogens and progesterone (feminine hormones). A third part of women suffer Dysmenorrhea or very painful menstruations. The dysmenorrhea is an irregularity of the menstrual function which is characterized by painful periods that disappear some hours before or with the menstruation and last 24 hours. The symptoms can be several; like presenting a similar pain to a stomach cramp but more intense and can go so far to be accompanied by a cramp in the legs, intense pelvic or abdominal pain, pain in the lower part of the back (at the height of the kidney), headache, nauseas, vomitings, faint, diarrhea or constipation, fatigue, as well as anxiety, irritability or depression. It’s to say, that affects as much the physical as the mind state. In some cases, besides the symptoms, are expelled during this menstrual period, clots of blood or endometrial molds. The main causes of the presence of menstrual colics is related to the hormonal imbalance, possibly to the excess of production of prostaglandins, hormones which emphasize the contractions of the uterine muscles; a second cause is the lack of care in the anal cleaning of some women, which cause that the feces get introduced in the woman’s vagina, developing secondary problems like the candidiasis and renal problems; this same cause is produced in the anal sexual relations and vaginals at the same time without having the care and the cleaning necessary. In the dysmenorrhea, used to be distinguished the following types:

a) Primary Dysmenorrhea:
Present in young women. In young women, the dysmenorrhea appears between 6 and 12 months following the menarche (first menstrual period) and usually it’s not due to any known disease, but to the effects of a hormone named prostaglandin, substance that belongs to the fat acids of 20 carbons (eicosanoids), which contains a clycopentane ring and constitutes a family of cellular mediator, with different effects and, often, opposed.
Some anti-inflammatories of free sale, such as the ibuprofen, the acetylsalicylic acid or the naproxen can help to relieve these symptoms. The hormonal treatment (contraceptives) also used to be effective, because it inhibits the production of hormones.
To decrease the symptoms it’s advised to realize exercise regularly (with a walk every two days, it’s enough); decrease the consumption of tobacco, alcohol and caffeine; apply heat to the zone and drink more liquids (water, juices, fruit, vegetables).
The primary dysmenorrhea tends to stay during the entire woman’s life; although, it’s used to decrease with the age and after the pregnancy. According to the statistics, they suffer it between the 30 and the 50 percent of women and used to be hereditary.

b) Secondary Dysmenorrhea:
The secondary dysmenorrhea appears in women older than 30 years, generally as a consequence of the endometriosis. The endometriosis is a relatively frequent disease which can affect to any women in fertile age, from the menarche to the menopause. The endometriosis can last until after the menopause. The endometriosis alters the quality of life of women who suffer it, affecting their relationships of couple, family, work and reproduction. The endometriosis consists in the appearance and growth of the endometrial tissue out of the uterus, above all in the pelvic cavity as in the ovaries, behind the uterus, in the uterine ligaments, in the urinary bladder or in the intestine. When the endometriosic tissue grows, appears lumps called tumors or implants that, the majority of the times, are benign or no cancerous and, rarely, they are related with the cancer. The implants of the endometriosis are influenced by the menstrual cycle, in such way that, in each menstruation, the woman who suffer endometriosis, the endometriosic tumors bleed, without possibility that the blood come out of the body, developing an inflammation and disorders in the urine. The pain is more continuous and heavy, being able to begin in a week before the menstruation and can be relieved or get worse during the menstrual cycle. In both cases, it’s convenient to go to the physician. The secondary dysmenorrhea also occurs by inflammation of the pelvis, cervical stenosis (narrowing of the uterus opening) and by the formation of tumors or cysts in the uterus. Finally, also the intrauterine devices can cause pain during the menstruation.

c) Medicines, medicaments or remedies for the menstrual colics
There are many medicines, medicaments or remedies for the menstrual colics (primaries or secondaries) that relieve, like analgesics of free sale like the acetaminophen, the aspirin, ibuprofen or naproxen. (The last 3 are anti-inflammatory medicaments). If you take anti-inflammatory medicaments like ibuprofen, it’s important that you do it as soon as the bleeding or the colics begin. If your periods are regular and can predict, when they are going to begin, start taking the anti-inflammatory medicament 1 day before the beginning of the period. That way, you can prevent in many cases the colics. To avoid the stomach in a mess caused by these medicaments, take the ibuprofen or the naproxen with the food or with milk.
If your symptoms are severe, it’s possible that you need a stronger medicament under prescription.
Also you can relieve the pain resting in the bed with an electric small cushion or a bag of warm water on the abdomen.
Some use for their treatment the contraceptive pills. Reduce the colics because decrease the production of prostaglandin. If the pills relieve you the pain, you can take them although you don’t need them as a contraceptive method.
